Moderate: use with caution

Can you have grapefruit with statins?

Also searched as: Zocor, atorvastatin, Lipitor, lovastatin

Short answer

With simvastatin or lovastatin, grapefruit juice should be avoided. Atorvastatin is affected less, mainly with large amounts. Some statins, like rosuvastatin and pravastatin, are not significantly affected.

Why it happens

Grapefruit blocks an enzyme (CYP3A4) that breaks down certain statins, raising their levels in the blood and the risk of muscle side effects.

What to do

  • Check which statin you take and ask your pharmacist about grapefruit limits for that one.
  • Follow the grapefruit guidance on your statin's label; the simvastatin label advises avoiding grapefruit juice.

Signs to watch for

  • Unexplained muscle pain, tenderness or weakness
  • Dark urine

If symptoms are severe, call 911 or go to the nearest emergency room.
